Remove class LexicographicVonMisesDissipationFunctional

Sander, Oliver requested to merge remove-lexicographicvonmisesdissipationfunctional into master

It stems from the old days when Dune couldn't properly handle vector types with heterogeneous blocking. Nowadays it is not used anymore. As it used old interfaces from dune-tnnmg that are about to go away, let's remove this old dissipation functional implementation as well.
